Congo’s Ebola Outbreak Tops 10,000 Cases as Vaccine and Conflict Hinder Containment

Congo’s Ebola outbreak surpasses 10,000 cases as conflict, funding gaps and the lack of a proven Bundibugyo vaccine complicate containment efforts.

The Democratic Republic of the Congo (DRC; République démocratique du Congo) had confirmed 10,060 Ebola cases, including 4,887 deaths, as of October 25, five months after the outbreak was declared, according to the Health Ministry's latest situation report, a case fatality rate of 48.6%. The World Health Organization (WHO) has described the epidemic as the largest Ebola disease outbreak ever recorded in the country, irrespective of virus species. The confirmed count alone is almost three times the 3,470 confirmed and probable cases recorded during the 2018-2020 epidemic in North Kivu, Ituri and South Kivu.

What sets this outbreak apart is the pathogen. It is caused by Bundibugyo virus, a species for which no approved vaccines or specific treatments currently exist, according to WHO. During the 2018-2020 response, an Ebola vaccine was licensed, two treatments were found to be highly effective and 303,905 people were vaccinated. None of those tools is approved for use against Bundibugyo. Official data from the ministry, WHO and its partners indicate that containment now rests almost entirely on contact tracing, isolation, safe burials and community trust, and that those measures are falling short in the insecure eastern provinces where the virus is spreading fastest.

The outbreak's chronology points to a late start for the response. Between April 24 and May 15, the epidemic moved from the detection of the first suspected cases to laboratory confirmation of Bundibugyo virus and the formal declaration of the country's 17th Ebola outbreak, the ministry's situation report says. The ministry reported that the index case was a nurse who died in a health facility in Bunia, the capital of Ituri province, and the European Centre for Disease Prevention and Control (ECDC) has said the outbreak was likely to have started many weeks before, given the number of cases and their geographic spread. WHO declared a public health emergency of international concern on May 17, and the Africa Centres for Disease Control and Prevention (Africa CDC) declared a public health emergency of continental security the next day.

The consequences of that delayed response are reflected in the outbreak's geographical spread. As of October 25, the virus had reached eight provinces and 66 of the country's 178 health zones, according to the ministry. Ituri remains the epicenter, with 7,211 confirmed cases and 3,370 deaths across 28 of its 36 health zones. North Kivu follows with 2,309 cases and 1,293 deaths, then Haut-Uélé with 436 cases and Tshopo with 83. Bas-Uélé, South Kivu, Sud-Ubangi and Nord-Ubangi have each reported 13 cases or fewer. Within Ituri, the Bunia health zone has recorded 1,949 cases and neighboring Rwampara 1,185.

Ituri's totals are the legacy of the early months; the newest cases point south, toward North Kivu. Of the 96 new confirmed cases reported on October 25, 58 were in North Kivu, including 35 in the Beni health zone alone. In its weekly situation report, WHO's regional office for Africa counted 1,466 confirmed cases nationally in the 21 days to October 19, a 10.4% rise on the previous three weeks. Cases in North Kivu grew 22.6% over the same comparison, against 3.7% in Ituri, and the agency said the latest seven-day average suggested "renewed intensification of transmission." Stéphanie Hoffmann, who coordinates the Ebola treatment center run by Médecins Sans Frontières (MSF) in Butembo, reached for a firefighting image. "It is like fighting a megafire," she said, adding: "As long as new hotspots continue to emerge while the virus spreads across northern North Kivu, containing the epidemic will remain extremely difficult."

Even these rising numbers may understate the epidemic, because they cover laboratory-confirmed cases only. ECDC has noted that the DRC's case and death data are under continuous review and harmonization, and it assessed that, given gaps in epidemiological information and limited follow-up of contacts, the outbreak is likely larger than currently reported.

If cases are being missed, deaths outside treatment centers are one place the gap shows. Of the 46 deaths reported on October 25, 40 occurred in the community, the ministry said. WHO has linked the high share of community deaths to challenges in early case detection and timely access to care, noting in its October 23 update that 307 of the 482 deaths reported since its previous update occurred in communities. The same WHO update put the case fatality ratios of the two previous Bundibugyo outbreaks, in Uganda in 2007 and in the DRC in 2012, at 30% and 50%. Children are dying at a disproportionate rate: as of September 30, those aged 17 and under made up about 27.4% of confirmed cases but 37.6% of deaths, UNICEF, the United Nations children's agency, reported. At MSF's Butembo center, 24 of 30 children treated between June and September died, the organization said.

Without a vaccine, early detection depends heavily on contact tracing. Yet the system is operating below its target. On October 25, responders saw 81.6% of the 19,853 contacts under follow-up, short of the 95% threshold, with North Kivu at 80.4% and Tshopo at 79.5%, according to the ministry. Africa CDC said on October 6 that more than 70% of new cases were still being detected among people who had not been identified and followed as contacts, an indication, in its words, that "transmission chains continue to escape the response system." WHO's regional office found that in the week to October 18, only 48.0% of investigated suspected cases were isolated and transferred within 24 hours.

The gaps extend into health facilities themselves, which have become a channel of transmission as well as a line of defense. By October 19, the outbreak had caused 301 cases and 48 deaths among health workers, according to WHO's regional office, which recorded infection prevention and control scores of 36.3% to 49.4% in repeated North Kivu assessments, against an 80% standard. In Butembo, Hoffmann said most children admitted over the summer had already been hospitalized for other illnesses, and that "it is highly likely that these children contracted the disease inside healthcare facilities," MSF reported.

Fixing tracing and infection control is harder still in eastern Congo, where insecurity compounds each weakness. WHO's regional office has warned that "escalating insecurity, displacement and restricted humanitarian access" are challenging operations and raising the risk of undetected transmission, and it reported that decontamination teams were assaulted in the Beni health zone on October 18. The ministry's latest report records that a screening point at Butuhe in North Kivu has been closed since a security incident on September 26, that the Avakubi checkpoint in Ituri is not functioning because of insecurity, and that physical attacks and threats against response teams in Tshopo's Bafwasende health zone led to a nurse being killed by members of the population. Part of North Kivu is under the control of the AFC/M23, and MSF sent a mobile team in October after confirmed cases emerged in Kayna and Alimbongo. "Part of the community does not fully believe that Ebola disease exists," said Nicodeme Kabuyaya, an MSF deputy project coordinator. Jean Kaseya, Africa CDC's director general, said in an interview that a 2025 outbreak in Bulape was contained in three months, but that insecurity, the humanitarian situation and "huge mobility of people due to the mining sector" had made this one more challenging to manage.

Insecurity is not the only obstacle to containment. The outbreak also lacks a vaccine approved for routine use against Bundibugyo virus, leaving vaccination dependent on research protocols. Ervebo, the vaccine licensed against the Zaire species of Ebola, is being used within research protocols after WHO's Strategic Advisory Group of Experts on Immunization concluded that evidence was insufficient to support programmatic use against Bundibugyo and that its efficacy in humans remains unknown, WHO's regional office reported. The DRC received an allocation of 70,000 doses on August 17. As of October 19, 8,497 people had been vaccinated across 15 health zones in five provinces, the same report said, and vaccination in Ituri had been temporarily suspended pending replenishment of stocks.

Whether Ervebo or a newer vaccine protects against Bundibugyo is the question a trial called Tumaini is meant to answer. The Phase 2/3 study, which uses ring vaccination of high-risk contacts, vaccinated its first participant in Bunia on October 23, according to the Coalition for Epidemic Preparedness Innovations (CEPI), which funds it. Ervebo is the first candidate to enter the trial, with Bundibugyo-specific vaccines under development by Moderna and by the University of Oxford with the Serum Institute of India expected to follow. "There is still no licensed vaccine against Bundibugyo virus, and the cost of that absence is being counted in the DRC every day, in lives lost and in health workers who go to work without protection," said Richard Hatchett, CEPI's chief executive. MSF has said that results from trials of Bundibugyo-specific vaccines are not expected for several months.

Drug research is at a similarly early stage. WHO's regional office reported 520 participants enrolled in the PARTNERS therapeutic trial and 307 in EBO-PEP, a post-exposure prophylaxis study that includes a randomized trial of the oral antiviral obeldesivir. WHO's temporary recommendations, updated in August, state that if effective vaccines and therapeutics become available, they "will not be a substitute for effective, core public health interventions."

All of this work, from tracing teams to trials, depends on funding, and money has become a further point of friction. The government's 180-day response plan carries estimated requirements of $1.3 billion, according to WHO's regional office. Africa CDC said it had helped mobilize about $2.9 billion for the response and backed DRC authorities' call for greater transparency on resources pledged, disbursed, received and used. "Every dollar mobilised for this outbreak must be traceable to the results it produces on the ground," Kaseya said. Individual agencies still report shortfalls: UNICEF had a $72.5 million gap in its $113.5 million plan as of September 30. In his interview, Kaseya listed gaps in human resources and community involvement, adding: "We don’t have vaccines and medicines."

The challenges facing the response are no longer confined to the DRC. The virus has crossed borders, raising the stakes for regional surveillance and preparedness. Counting the DRC, the outbreak has affected five countries. Uganda recorded 20 confirmed cases and two deaths before WHO declared its outbreak over on August 27, and France reported one case in a doctor returning from the DRC, ECDC said. As of October 26, Kenya had reported nine cases, including two deaths, all linked to a Kenyan citizen who had been living in the DRC and traveled to Kenya via Uganda. A woman confirmed with the virus in Abuja, Nigeria, after traveling from Uganda was medically evacuated to Switzerland on October 26. WHO assesses the risk as very high in the DRC, high for neighboring countries and low regionally and globally, and said that assessment is being revised. It continues to advise against restrictions on travel or trade. Africa CDC convened African health ministers on October 28 to assess the situation.

Chikwe Ihekweazu, executive director of WHO's Health Emergencies Programme, has said that a successful Tumaini trial "would be the first time we have clear evidence of vaccine efficacy against Bundibugyo virus." By MSF's estimate, results for the Bundibugyo-specific candidates are still several months away. The question the official data leave open is whether contact tracing, isolation and safe burials, the measures now carrying the response, can be brought up to standard in North Kivu's conflict zones quickly enough to turn the curve before any vaccine is proven to work.
